Consent to personal data processing

1. General Provisions By filling out forms on the website https://supplementopt.tilda.ws/, I, the personal data subject, freely, of my own will and in my own interest, hereby provide my consent to LLC "Supplement" (PSRN/OGRN 123200009108, TIN/INN 2012010945, address: 1/8s21 4th Syromyatnicheskiy per., Moscow, Russia; hereinafter referred to as the "Operator") to process my personal data in accordance with Federal Law No. 152-FZ "On Personal Data" and Roskomnadzor Order No. 140 dated June 19, 2025.

2. List of Personal Data To achieve the goals of interaction, the Operator processes:

• Surname, first name, and middle name (if any);
• Contact details (phone number, email address, Telegram account);
• Delivery address, place of residence/registration;
• Identity document details, TIN (to the extent necessary for concluding a contract);
• Bank details and tax status information;
• Technical data collected by metric programs (including Yandex Metrica): IP address, browser data, device type, operating system, location details, referral sources, as well as information about behavior on the Website (including the use of "Session Replay" / "Webvisor" technology);
• Other data provided voluntarily within the framework of interaction with the Operator.

3. Purposes of Personal Data Processing

• Processing of incoming applications, orders, and inquiries;
• Providing feedback and support;
• Conclusion and execution of contracts;
• Compliance with legal requirements in the field of civil law relations;
• Maintaining statistics and analytics using depersonalized data;
• Organization of order delivery involving third-party courier and transport services;
• Analysis of marketing activity effectiveness and improvement of the Website performance using web analytics systems.

4. Personal Data Actions The Operator is entitled to perform collection, recording, systematization, accumulation, storage, clarification (update, change), use, transfer (provision, access), depersonalization, blocking, deletion, and destruction of personal data, with or without the use of automated tools.

5. Depersonalization of Personal Data Depersonalization is carried out using methods compliant with Roskomnadzor Order No. 140, excluding the possibility of restoring information without additional data. Original and depersonalized data are stored separately. Access to depersonalization methods and logs is restricted.

6. Storage Period Personal data are processed until the goals of processing are achieved or until consent is withdrawn. Upon expiration of the period, the data shall be destroyed or depersonalized within the timeframes established by law.

7. Withdrawal of Consent Consent may be withdrawn at any time by sending a notification to the Operator's physical address or to the email address: info@supplementopt.com. Withdrawal does not affect the lawfulness of processing prior to the receipt of the notification by the Operator.

8. Transfer to Third Parties I hereby consent to the transfer of my data to the following third parties:

• YANDEX LLC (16 L. Tolstoy St., Moscow, 119021, Russia) — for data processing in the Yandex Metrica system for web analytics purposes;
• Transport and courier services — for order delivery purposes;
• Authorized government bodies — solely on the grounds provided by the legislation of the Russian Federation.
